The CommunityCare Longevity Program rewards members who are committed to improving and tracking key health metrics. If you’re ready to challenge yourself and stay accountable to your long-term health goals, this program is for you.

How It Works:

  • Eligible members (Primary Account Holder and Spouse) must complete at least one of the three approved health tests between October 1, 2025 and January 31, 2026.

    DEXA Scan

  • Use the CommunityCare app to upload your test results and receipts. Submissions are to be received by January 31, 2026 to count toward a discount. Tests are eligible for crowdfunding with a limit of 2 tests per eligible adult. Crowdfunding varies by type of test.

  • Your account’s discount is based on how many test each adults completes and passes. Discounts begin in January 2026 (or February if you submit in January).

  • All members who qualify will receive a badge on their CommunityCare account.

FAQs

What if my DEXA scan doesn’t measure visceral fat?

Your score won’t count. You’ll need to use a facility that includes visceral fat in the report.

Can I use old test results?

No. Only tests completed and submitted between 10/1/25 – 1/31/26 will be accepted.

Do I need to do this every year?

Yes. Discounts apply only for the following calendar year, and you must re-certify annually.

Can the Longevity Discount be combined with an active referral or promo code?

The Longevity Discount cannot be used at the same time as a referral or promo code. It will automatically apply once your current referral or promo code expires.

Can I take the tests again if I fail the first time?

Yes, however, CommunityCare will only crowdfund up to 2 tests per eligible adult, per year. You are responsible for the costs of any additional tests.

Does my spouse have to participate?

If your spouse is on your account, yes. To receive a discount, both the primary account holder and spouse must participate.

Can a dependent do it in my place?

No. Only the primary account holder and spouse (if applicable) can participate.
